I Hereby authorize Consolidated Electrical Distributors, Inc. (CED) to make an
electronic deposit of my payments via ACH (draft) or other Electronic Funds
Transfer (EFT) when presented with a check (physically or by fax or phone)
from Company/Customer below.
2.
I Certify that I am a signer on the account listed below with the authority to
grant this authorization on behalf of Company/Customer.
3.
I Hereby request, authorize and direct Company/Customer’s bank to honor and to
treat as authorized checks, drafts or funds drawn in Company/Customer’s name
in accordance with this authorization.
4.
I Certify that in the event that any such draft or EFT is returned unpaid, I agree
to have the account debited electronically (drafted) for an item fee of
$25.00, plus any applicable taxes.
5.
I Certify that this authorization shall remain in full force and effect and the
authority herein given to CED shall remain until CED receives written notice
of revocation of such authority. Revocation shall not effect any action taken
prior to receipt of such notice.
6.
This form supercedes any
and all previous authorizations.
